Nurse-led advice and care coordination
Because Aurora is led by a registered nurse, someone experienced is always keeping an eye on the bigger picture.
What a care coordination visit includes
- Carers record any concerns
- Kirsty or Kayleigh review them and talk to the family
- Kirsty advises on next steps, such as asking for an occupational therapist or physiotherapy referral
- Where needed, Kirsty speaks to professionals directly
How often, and for how long
Health and wellbeing are monitored on every visit, not just at reviews. Plans are reviewed monthly, with regular contact by text or email each week.
A real example
One client was struggling to move from her wheelchair to her riser recliner. We suggested involving an occupational therapist, who arranged a suitable transfer aid.
Areas we cover
North Dorset, South Somerset and South Wiltshire. Call Kirsty with your postcode if you are not sure.
